Organization Library Card Application Form
An Organization Library Card from the Madison Public Library allows your after school program, preschool, daycare, or school to check out library materials for use in your classrooms. The organization is responsible for all materials checked out by authorized staff/users. Apply for an Organization Library Card at Madison Public Library by filling out the form below. Once the form is submitted, a new library card will be mailed to you. Please note that if you apply online, you will not be able to use your new card until you receive it in the mail. If you would like to use your card the same day you apply, please visit your local neighborhood library with your photo ID and proof of current address and apply in person.
PLEASE NOTE: Applying for an Organization Library Card requires you to submit a copy/photograph of a valid photo ID and Proof of Authorization (such as a letter from the principal/head of organization). Once your application has been processed, Library staff will delete any copies/photographs submitted.

Madison Public Library cards are only available to people live in the Madison, WI area or the South Central Library System. If you live outside this service area, please contact your local library to apply for a card.
Why does this page look different?
